Submit a Report
When submitting a report, you will be asked to provide detailed information regarding the incident you are reporting.
Speak Up reports are not monitored 24/7. The information you submit will be reviewed by a Notre Dame administrator within two University business days (Monday - Friday, 8 a.m. - 5 p.m.). If this is an emergency, please call 911. Information provided through Speak Up is not anonymous.
Please select one of the following to submit a report:
- I am reporting an incident of racial or discriminatory harassment.
- I am reporting an incident of sexual harassment, assault, misconduct, dating or domestic violence or stalking.
- I am reporting an incident of retaliation or a violation of University-issued no-contact order.
- I am reporting an incident of initiation and hazing.
- I am reporting an incident other than those described above.
